KISS Map: Find It Before You Rebuild It

Agents duplicate code because they cannot cheaply answer "does this already exist, and where?" This builds a durable index and uses it instead of grepping blindly or reading the whole tree. The useful part is the twin check. Searching by name misses duplicates that were named differently, so this checks by question: what does this code answer, and does something already answer it? It indexes concepts rather than only symbols, which is what makes that check possible.
