Agent Reliability Forge
Example session with this skill installed
We have a customer support agent that uses tools for order status, refunds, and shipping. It works in demos but occasionally invents order statuses or gets stuck in clarification loops. Run the full Agent Reliability Forge process and give a clear production readiness assessment.
- Read your context and instructions
- Compiled the agent reliability forge
- Generated the document
Summary
Applied the full 6-phase Reliability Forge to your customer support agent. Produced a formal Reliability Contract, prioritized Failure Mode Catalog, evaluation strategy, telemetry requirements, self-correction design, and a clear Production Readiness Gate status.
1. Reliability Contract (v1.0)
Guarantees
- Never invents order status, refund eligibility, or policy details. All factual claims must come from tool results.
- Always cites the specific tool output used for any factual claim.
- Escalates to human when confidence drops below calibrated threshold or user expresses frustration twice.
Refusals
- Never processes refunds or account changes without explicit human confirmation for amounts > $50.
- Never shares internal notes or other customers’ data.
Success Criteria
- ≥ 92% resolution rate without escalation on golden set
- Silent failure rate (wrong but confident answer) < 1.5%
- p95 end-to-end latency ≤ 12s
- Average cost per resolved ticket ≤ $0.18
2. Failure Mode Catalog (High/Critical only)
| Mode | Likelihood | Impact | Detection | Mitigation |
|---|---|---|---|---|
| Hallucinated order status | Medium | Critical | Grounding check vs order tool | Force citation + refuse if no tool result |
| Tool result ignored | Medium | High | Post-tool utilization audit | Require tool result in final answer |
| Infinite clarification loop | Low | High | Turn count + state hash | Max 3 clarifying questions then escalate |
| Policy violation on refund | Low | Critical | Pre-action policy gate | Hard block + human confirmation |
3. Evaluation Strategy
- Golden set: 120 real + synthetic tickets
- Adversarial probes for every High/Critical mode above
- Continuous: Shadow 10% of live traffic, weekly scoring against production + human labels
4. Required Telemetry
Every run must emit: run_id, contract_version, outcome_class (Success/Degraded/Failed/Escalated), tool_calls sequence, token/cost metrics, degradation signals.
5. Self-Correction Design
- Bound retries to 2 attempts with tightened constraints
- Circuit breaker after 3 consecutive failures of the same class
- Controlled escalation package that includes full tool history and contract version
6. Production Readiness Gate
| Item | Status |
|---|---|
| Reliability Contract exists & versioned | ✅ |
| Failure Mode Catalog covers High/Critical | ✅ |
| Evaluation harness designed | ✅ |
| Telemetry schema defined | ✅ |
| Escalation paths designed | ✅ |
| Cost & latency budgets defined | ✅ |
| Kill-switch / rollback procedure | ⚠️ Needs implementation |
| On-call ownership assigned | ⚠️ Pending |
| Drift detection scheduled | ⚠️ Pending |
Verdict: Not yet production-ready. Two operational items remain. Once those are closed, the agent can be authorized.
Next steps
- Implement the kill-switch and assign on-call ownership
- Schedule the first continuous evaluation job
- Re-run the Production Readiness Gate
- Authorize production traffic only after all items are green

What you get
About this skill
Most AI agents work in demos and fail silently in production. Teams waste hours reviewing outputs, chasing hallucinations, and guessing whether an agent is actually ready to ship.
Agent Reliability Forge gives you a complete, repeatable reliability process in one skill:
• Formal Reliability Contract (Guarantees + Refusals + Escalation Policy) • Agent-specific Failure Mode Catalog with detection & mitigation • Evaluation harness design (golden + adversarial + continuous) • Structured telemetry requirements • Self-correction loops with circuit breakers • Hard Production Readiness Gate that tells you clearly “ready” or “not ready”
Instead of vague advice, you get concrete engineering artifacts you can use immediately.
Perfect for developers and AI engineers who need to ship agents with higher confidence and less manual babysitting — without enterprise pricing or consulting overhead.
One skill. Clear process. Production-grade outputs.
How to install
Works the same in every agent - Claude, Cursor, Codex, Copilot and 20+ more.
- 1
Download the ZIP
Free skills download straight away. Paid skills unlock right after purchase.
- 2
Unzip into your skills folder
Every agent reads skills from one folder on your machine. Drop the unzipped folder in there.
- 3
Ask your agent to use it
Restart the agent if it was already running. It picks the skill up automatically - no config needed.
